Below you will find documents and forms we will need for you to fill out and return to us if you want to participate in one of our Team trips.

The forms are electronic. There are two steps involved in using them: the first is to fill out the information, the second is to return the completed form(s)to us.

The form(s) may be filled out and submitted electronically using Acrobat Reader. After filling in the required information you have a choice in how you submit the information. You can either click on the “Print” button at the top of the page, mailing us the printed output. Or, you may submit the completed form electronically by clicking on the “Submit by Email” button, also at the top of the page.

Alternatively, the form may be printed and filled out by hand, and then returned to us by mail. This option may be required when more than one person is required to provide information on the form.
